Horizon Family Medical Group is aware of the potential impact of the current coronavirus crisis on our patients’ physical health and emotional wellbeing. We will all get through this together. Our goal is to provide a forum where we can support each other during a time of uncertainty. A place to share concerns about loved ones, job loss, financial difficulties, isolation, loneliness, and fear. This forum is for you, our Horizon patient family. This forum is free. We hope you join us!
Horizon Family Medical Group would like to invite you to our Patient Support Circle on Zoom. Zoom is a HIPAA-compliant platform that allows patients to meet virtually to offer support to each other. This support circle will be cofacilitated by Patt Smith, LCSW and Lyndsay Brischoux, LCSW.
We will be meeting daily, from 11:00 AM to noon (ET). This is a group session with other Horizon Family community members. Please be mindful that the things you say will be shared with the group. We ask that individuals not share personal identifying information. If you choose to join us, we ask that you make a conscious effort to maintain confidentiality and set up a private space in your home.
In an extra effort to maintain safety, you will be able to see the faces of the facilitators but we ask that you only participate with the audio option of this platform.
